Which one is correct icc profile in Photoshop for convert CMYK to Lab, Or where can i find the formula?

for example:
C:100, M:0, Y:0, K:0 --> L:58, a:-41, b:-54

In Photoshop go to Edit > Convert to Profile and select Lab Color in the Destination Space.

As regards the formula, there is no formula to convert CMYK to Lab. The conversion is made by means of a table.

There is no formula for CMYK. ICC profile itself is the “look-up table formula”. Then it depends on CMYK standard and on rendering intent used.
So there are many results, at least:
Number_of_standard_profiles x 5 rendering intents
